We work with your insurance to help get your prosthetic paid for.

We want to make sure all of our patients have the prosthetic eye they need, and help covering the cost.

Here is the information we need from you:

  • Your full name

  • Your date of birth

  • The address on file with your insurance company

  • The name of your Primary Care Physician or Eye Doctor

  • Your insurance member ID

If you’d like, you can contact your insurance company (the number is usually on the back of your insurance card) and ask them:

  • What is my coverage amount for Durable Medical Equipment?

  • What is my deductible, and how much have I met for this year?

  • Do I need a prescription or referral from my doctor? If they say yes, you will need to contact your doctor before you visit our office.

 

Your insurance may cover some or all of the cost of your prosthetic eye and its maintenance. We work with insurance companies to make sure you get the maximum benefit. We are in-network with many local insurance companies, and we are often able to work with other companies for coverage, because there are so few ocularists in the country.
